Eligibility, Limitations & Enrollment

ELIGIBILITY

1. Full time employees (30 hours per week), and their Spouses, at least age 19 and not yet 65 as of their initial effective date.
2. Dependent Children under the age of 19, or 23 if full time student primarily dependent on the employee for support.

LIMITATION ON U.S. COVERAGE
Initial departure from the US must occur within 30 days of the employee's initial effective date. Coverage terminates on the earliest of the following dates:
1. The date the employee ceases to be an active full time employee; or
2. The date the employee becomes eligible for any other medical benefit plan, whether or not the employee elects to become covered under such plan; or
3. Twelve months following return to the US.

PRE-EXISTING CONDITIONS
A pre-existing condition is any Injury, Illness (including Pregnancy), or Mental or Nervous Disorder, including complications and consequences associated with these conditions, for which diagnosis or treatment was received or prescription medications or drugs were taken or for which a reasonably prudent person would have sought diagnosis and/or treatment during the 90 days just prior to the employees or dependents initial effective date. Benefits for Pre-existing Conditions are covered after six months during which no treatment (including prescription medications or drugs) is recommended or received, or after twelve months of continuous coverage under the Indy+Plus plan, whichever first occurs.

TAKEOVER PROVISION
This provision allows you to implement Indy+Plus without requiring that your employees start over on waiting periods for Maternity, Mental and Nervous, Wellness and Pre-existing Condition benefits.

UNDERWRITING REQUIREMENTS
For groups of 10 or less employees, Evidence of Insurability is required for each employee. For groups of 11+ employees, Simplified Evidence of Insurability is required for each employee. The Company, at its sole discretion, may waive these requirements if claims experience is provided.

COBRA
Medical insurance continuation is available as required by U.S. law.
